Adoption Form

Best Friends, a 501(c)3 non-profit corporation, and the undersigned (“Adopter”) in consideration of the mutual promises in this Agreement, intend to be legally bound by the Agreement.

NO REPRESENTATIONS. Adopter understands that Best Friends does not guarantee the health, temperament, or training of animals for adoption, and the Adopter hereby holds Best Friends of Baker, Inc., its agents, volunteers and employees harmless from any claim, cause of action or liability for any injury or damage to the persons or property in connection with the adoption or ownership of the animal.

In order to adopt one of our animals, you must:

  1. Be 21 years of age and have proper identification.

  2. Provide written consent of pet ownership from your landlord (if you are a renter).

  3. Agree to spay or neuter the animal you are adopting (if not already spayed or neutered).

  4. Provide care of the animal. Adopter understands that the animal being adopted is an indoor and or indoor/outdoor pet and agrees to allow it appropriate access to the home. Adopter agrees to provide the animal with fresh water, wholesome food, adequate exercise and affection. Adopter agrees not to tie up or chain the animal or to allow it to ride uncaged in the back of a pick-up truck or other open vehicle. Adopter also agrees to comply with local and state statutes and ordinances, including licensing and leash laws. Adopter agrees to microchip and provide a name tag with contact information to prevent theft or loss of the animal. We recommend keeping the Best Friends tag on your animal until you obtain your own identifying tag, it allows us to reunite you with your animal if you become separated.

  5. Veterinary care. Adopter agrees to provide the animal with the necessary vaccinations as advised by a licensed veterinarian. Adopter agrees to obtain immediate veterinary care should the animal become sick or injured, and to take full responsibility for all veterinary expenses.

  6. Declawing (felines only). Adopter agrees NOT to declaw the cat, understanding that the process of declawing is a painful amputation of the first knuckle, which renders the animal defenseless and inflicts unnecessary pain.

  7. Adoption Fee: Adopter agrees to pay the agreed upon amount determined by Best Friends which covers expenses incurred by the animal while in our care and helps us to care for the next animal in need. The adoption fee is tax-deductible. Checks are accepted and should be made out to Best Friends of Baker, Inc.

Best Friends reserves the right to refuse adoption to anyone. No animal will be adopted to persons who have a history of animal abuse or neglect. No animals will be adopted to prospective owners who mislead the organization or fail to provide accurate information on the adoption application. No animals will be adopted to parties with the intent of research or breeding.

AGREEMENT: If I, (Adopter), am unable to maintain custody of this adopted Animal due to financial or other hardship, I agree to return the animal to Best Friends of Baker, Inc., in Baker City, Oregon. I agree NOT to give this animal to another private party or shelter unless permission is granted after consultation with Best Friends.

Best Friends is an all-volunteer, foster-based rescue, and our ability to take animals returned for behavioral reasons (aggression towards humans or other animals) is extremely limited. Should you encounter behavioral problems with the dog after adoption, we recommend reaching out to an experienced dog training or animal welfare professional for help.
